We bill the following insurance companies: BlueCross BlueShield, Premera Blue Cross, Lifewise, Cigna, HealthNet, Providence, ODS, United HealthCare, Aetna, PacifiCare, PacificSource, Kaiser, and Auto Insurance Companies.

We accept payment in the form of cash, check, Visa and Mastercard. 
There is a 20% discount for payment in full at the time of service.

Hours: Monday-  Friday, evenings and Saturdays by appointment only
